On a scale from 1 to 5 (1=needs work and 5=all good)
Two options at the most can be selected, one is the grade above (the number from 1-5) and the other is the remark(e.g.  the volunteer's voice is low and she should keep her eraphone closer). You may also just choose one number for each question and leave "remark" blank if there is no specifics.
